As an Indiana native, Dr. Mullendore was consequently accepted into Indiana University School of Dentistry. Excelling in dental school, he graduated with distinction in 2003, ranked 5th in his class and was inducted into the Omicron Kappa Upsilon Dental Honor Society. In the year following graduation from dental school, he completed a General Practice Dental Residency at Wishard Hospital in Indianapolis, Indiana.
